Practice & Membership
At its core, Distinctive Women's Health Care is for women who want a physician who truly knows them, takes ownership of the full picture, and stays involved every step of the way.
I'm a Yale-trained OB-GYN with more than four decades of experience, and I provide full-spectrum women's healthcare across fertility, midlife, menopause, and beyond. For many of my patients, I'm both their OB/GYN specialist and the doctor they call first when something feels off. I look at the whole picture — hormones, sleep, mood, sexual health, breast cancer risk, prevention, and longevity — because those pieces are connected, and they deserve to be treated that way.
You’ll find that my practice is organized, accessible, and care feels deeply personal. Appointments run on time, visits are never rushed, and your care is followed through with the same level of attention whether you're sitting across from me or checking in between visits.
I see myself as a primary care specialist for women's healthcare, with the added benefit of OB/GYN specialist expertise. That combination is not common, and it matters.
Most women in midlife end up with one doctor for hormones, another for sleep, another for breast health, and no one is truly connecting the dots. In my practice, I connect those dots. I manage prevention, risk assessment, perimenopause and menopause, emotional health, sexual health, fertility, and long-term health planning. I find where they fit together, with the time and continuity to do it well. If something is outside my scope, I'll refer you to the right specialist and stay involved every step of the way.
Not in the ways that matter most. I've practiced concierge-style medicine for many years and haven't taken insurance for 25 years. If you're used to having time with me, being able to reach me, and feeling like I truly know you, that part is not changing.
What is changing is what I'm able to offer you. This evolution adds more structure and more services: more thorough screening, more proactive follow-up, and a broader set of support built into your care experience. This is an upgrade, not a departure.
A membership-based physician practice works on a subscription model where patients pay a fee for access to a range of healthcare services that go beyond typical insurance-covered care. Some of these services include:
Annual wellness exams.
Each member begins with a personalized annual wellness exam designed around their individual health needs and long-term goals. This visit takes a comprehensive, longevity-focused view of your health to address physical well-being, hormonal transitions, sexual function, emotional health, and perform preventive screenings. In addition to a Pap smear and routine labs, prophylactic cancer screenings for breast, uterine, ovarian and cervical cancers are performed when needed. This visit sets a clear, informed foundation for the year ahead, allowing us to identify concerns early and create a personalized plan so care is proactive and deliberate, not reactive.
(Note: third-party services, including diagnostics and labs, are not included in membership fees but may be reimbursed through insurance depending on your plan.)
Access to in-person and virtual visits for medical and wellness services.
Care is offered both in person and virtually, allowing you to receive support in the way that best fits your needs throughout the year.
In the office, we provide diagnostic screenings and therapeutic treatments that may be used over time as part of a coordinated plan. Some of these include Morpheus/v-tone treatments, Emface treatments, and weight loss management programs.
You’ll have the flexibility of up to 8 follow-up visits to monitor progress, adjust treatment, and stay ahead of changes without feeling rushed or delayed. You’ll also have direct access to the practice, with same-day responses for urgent questions and timely appointments when symptoms shift, so support is there when you need it most.
Direct access to care coordination.
My practice team will support with care coordination: medical record retrieval, scheduling visits with specialists, and courtesy billing for out-of-network services. When surgical care or additional specialists are needed, referrals and coordination are handled for you, and members receive 50% off professional fees on procedures not included in the membership.
After-hours virtual urgent care and other member services.
I have partnered with Monarch to provide services that remove the friction women often experience with today's health system. Members have access to after-hours virtual urgent care, medical record retrieval services, scheduling support, and courtesy billing for out-of-network services provided.
As a member, you'll have direct access to me, whether in person or via telehealth. A full list of services covered under the membership fee is available by contacting membership@drjeffreykotzen.com.
Enrollment in my practice is intentionally limited so I can provide personalized, high-quality care to each member. Once the practice reaches capacity, a first-come, first-served waitlist will be established.
If you decide to enroll, you can either click the enroll button on my website or call to enroll through a conversation with my membership team.
Daughters (up to age 26) of members are welcome to see Dr. Kotzen. Fee-for-service pricing will apply. Please contact the practice for more details.
